New Patient Forms

To expedite your visit to PARC Urology, we invite you to fill out our new patient forms before you arrive. These will ask about the current concern, as well as past medical conditions and surgeries, medications and allergies, and any medical problems that run in the family.

New Patient Visits

We invite you to fill out our new patient forms before you arrive, or you can do this in our waiting room on our patient i-pads.

All new patients will be weighed and have their height measured. Children who are toilet trained should not urinate within an hour or so of their scheduled arrival as we may ask for a urine sample.

Our specialists are committed to on-time service. Please use our free wi-fi service in the waiting room and watch the kids enjoy coloring our special old-fashion chalk walls in each examining room!

If your child has had x-rays made, please bring copies with the actual pictures, as well as reports, with you for our doctors to personally review.

Insurance

We accept most commercial insurance plans, as well as Medicaid and CHIPs.
